In the message section please include the following information:

• date of referral

• patient's name, address, date of birth and contact number

• any relevant medical history

• details of the treatment being referred

Please email any radiographs to contact@themanchesterfaceclinic.co.uk or alternatively you can give copies to your patient to bring with them.

Patient's being referred to our clinic will require an initial consultation with Dr Macdonald, the fee for this consulation is currently £55. The patient will then be provided with a detailed treatment estimate outlining the costs of any purposed treatment.

We will not seek to register any patients referred to our clinic and will make no attempts to encourage them to join our practice.
